Students are required to use the Case of Antwon Fisher from the movie, The Antwon Fisher Story to respond to the questions. Please use essay and expository writing format in responding to all the questions. This is an independent assignment and should NOT be discussed with other students, field instructor, or other behavioral practitioners, until after final grades are submitted.
1. Presenting Problem: Include a summary of the problem that led the client to seek services. Please include the symptoms, duration, and impact that the problems may have on the client’s functioning (social, family, academic, employment, relationships, etc.). Refer to the HUSSW Case Assessment guide to assist with writing this section. Weight: 20%, Minimum 2 pages – maximum 3 pages.
2. Clinical Desсrіption of the Client: All the following categories must be completed. Provide the rationale for the selected diagnoses. Indicate examples of the four D’s – distress, deviance, dysfunction and danger of the client’s behavior/diagnoses. Explain the reason these symptoms meet the criteria of the four D’s. Weight: 40%, Minimum 5 pages – maximum 6 pages.

3. Theories of Causation: Discuss two theories that show the most promising explanation about each of the client’s diagnosis (all clinical and personality disorders). 4. Treatment: Explain at least two interventions that have produced the best results in alleviating or minimizing the client’s symptoms. Explain how these interventions address the diagnoses. Weight: 10%, this section is 2 pages required.
