I’ve attached an evaluation note that another writer attempted. (Peter, you weren’t available for some reason. And I know you can fix this.) With multiple amendments, it is still not where it needs to be. There is too much unnecessary substance. There is no ROS. The differential diagnosis section is not correct. Does not follow the DSM5. Reflections is not good. And is missing the treatment plan. Follow the template provided. Gives everything that needs to be included. The evaluation is on an 18 year old who has a conduct disorder.
Most of it just needs to be rewritten. I will attach a previous assignment for a guide along with the template and example provided from the class.
